Betel Leaf Benefits: What the Leaf Offers, and Why Paan Is a Different Question

Betel leaf has been chewed, brewed and applied across South and Southeast Asia for well over two thousand years. It appears in Ayurvedic practice, in temple offerings, at weddings and at the end of meals.

It is also at the center of one of the more serious public health problems in the region—not because of the leaf, but because of what is usually wrapped inside it. Getting that distinction right is the most useful thing anyone can tell you about betel.

The Leaf and the Quid Are Not the Same Thing

Almost every claim you will read about betel—good and bad—comes from confusing these three. The leaf itself has a genuine traditional pharmacology. The nut is the problem.

What Is in a Betel Leaf

Piper betle belongs to the same family as black pepper and long pepper, and like them its value sits in an aromatic essential oil rather than in any nutritional contribution.
Reviews of the Piper genus catalogue the leaf's main constituents as phenolic compounds including chavibetol, eugenol—the same compound that gives cloves their character—and hydroxychavicol. These are responsible for the sharp, peppery, faintly medicinal taste, and they are also where the antimicrobial activity observed in laboratory work comes from.
The leaf is eaten in very small quantities, so it makes no meaningful contribution of calories, protein or vitamins. It is used the way a spice is used.

Traditional Uses

In Ayurveda, tambula—the betel leaf—is described as pungent, warming and drying, and is used to stimulate digestion, freshen the mouth and clear congestion. The custom of offering paan after a meal comes directly from that: the leaf was understood as a digestive.
  • After meals, chewed alone or with fennel and cardamom, as a digestive and breath freshener
  • For coughs and congestion, where warmed leaves or leaf juice with honey is a long-standing home remedy
  • Applied to the skin, traditionally to minor wounds and boils, drawing on the antimicrobial activity of the oil
  • Warmed and applied to the chest for congestion in children, a practice still common across India
  • In ceremony, where the leaf carries meaning quite separate from any medicinal use
Much of this rests on centuries of practice rather than on clinical trials. The laboratory work on the leaf's antimicrobial and antioxidant activity is reasonably consistent; human trials are scarce.

The Areca Nut Problem

This needs to be stated plainly, because it is frequently softened.
In 2003 the International Agency for Research on Cancer, the World Health Organization's specialist cancer agency, reviewed the evidence on betel quid and areca nut chewing. It had already concluded in 1985 that betel quid with tobacco causes cancer in humans. The 2003 review extended that conclusion: chewing betel quid without tobacco is also carcinogenic to humans, and so is areca nut on its own.
The cancers involved are of the mouth, pharynx and esophagus. Areca nut is also the principal cause of oral submucous fibrosis, a progressive stiffening of the lining of the mouth that gradually restricts how wide a person can open their jaw and carries a substantial risk of turning cancerous. A 2004 study in Oral Oncology found betel quid without tobacco was a risk factor for oral precancerous lesions in its own right.
There is genuinely good news alongside this. Meta-analyses published in 2022 and 2025 examined what happens after people stop, and found that the raised risk of oral, pharyngeal and esophageal cancer falls following cessation. Stopping helps, and it helps measurably.

What This Means in Practice

The practical position is not complicated:
  • The betel leaf on its own is not the concern. Used as a herb—chewed occasionally, brewed, applied to the skin—it carries the risk profile of a strongly aromatic culinary leaf.
  • Areca nut is the concern, whether or not tobacco is added, and whether or not it is wrapped in a leaf.
  • Commercial paan masala and gutkha typically contain areca nut and often tobacco, and are the products most strongly associated with harm.
  • Sweet paan and meetha paan still usually contain areca nut. The absence of tobacco does not make them safe.
If you enjoy paan and want to keep the part that is traditional and harmless, the leaf with fennel, cardamom, rose petal preserve and coconut—and no supari—is the version worth keeping.

Using the Leaf Without Areca Nut

  • As a digestive after meals. Chew a fresh leaf with fennel seeds and a cardamom pod.
  • As a tea. Steep two or three washed leaves in hot water for five minutes, with honey and ginger if you like.
  • For a cough. Warmed leaf juice with a little honey is the traditional preparation.
  • As a wrap. The leaf works as a wrapper for savory fillings much as a vine leaf does.
  • Topically. A clean, lightly crushed leaf on a minor graze is an old remedy, though it does not replace proper wound care.
Wash leaves thoroughly, as they are grown close to the ground and are usually sold fresh and unwashed. Store them wrapped in a damp cloth in the refrigerator and use within a few days.

Who Should Be Careful

  • Anyone who currently chews betel quid, for whom the useful conversation is about stopping the areca nut rather than about the leaf
  • Pregnancy, where areca nut use is associated with poorer outcomes and should be avoided entirely; the leaf in culinary amounts is a separate matter worth raising with your own doctor
  • Anyone with mouth ulcers, restricted jaw opening or a white or red patch in the mouth, which should be examined by a dentist or doctor promptly rather than treated at home
  • Children, for whom areca nut in any form is unsuitable
If you have chewed betel quid regularly for years, a dental examination is worth arranging even if nothing hurts. Oral precancerous changes are usually painless and are easiest to treat when found early.
