Black Coffee with Honey: What Each Part Does, and What the Evidence Shows

Black coffee with honey circulates widely as a morning ritual, usually attached to a claim about weight loss or metabolism. It is a pleasant drink and there is nothing wrong with it.
The two halves of it, though, sit in very different places evidentially. Coffee is one of the most studied things people consume. Honey has been studied too, and a recent and rather large review produced findings that are not what most people expect.
The Coffee Part Is the Well-Supported Half
Coffee has been followed through enormous populations over decades, and the picture that has emerged is consistently favorable.
A dose-response meta-analysis published in the European Journal of Epidemiology examined coffee consumption against all-cause, cardiovascular and cancer mortality, analyzing smokers and non-smokers separately—an important distinction, because heavy coffee drinkers were historically more likely to smoke, which had confounded earlier research. A later dose-response meta-analysis looked at caffeinated coffee across a range of health outcomes in the United States population.
Moderate coffee intake is also associated with lower risk of type 2 diabetes and of liver disease, including hepatocellular carcinoma, across multiple pooled analyses.
These are observational findings, so they show association rather than proof of cause. But the consistency across many populations and outcomes is what makes coffee unusual: the honest summary is that moderate coffee drinking looks either neutral or mildly beneficial for most people, which is more than can be said for most habits.
The Honey Part Deserves a Closer Look
Honey has a reputation as the healthy sweetener, and there is something to that—it is a whole food with trace enzymes, minerals and polyphenols that refined sugar does not have.
It is also, in composition, mostly fructose and glucose. And a 2025 umbrella review published in Nutrition and Diabetes brought together 69 randomized controlled trials covering 3,544 participants to look specifically at dose.
Its finding on honey was mixed and, in parts, unflattering. Ten grams of honey a day—about a heaped teaspoon and a half—was associated with a possible reduction in glycated hemoglobin, the three-month measure of average blood sugar. But the same intake was associated with adverse effects on systolic blood pressure, aspartate transaminase (a liver enzyme), triglycerides, fasting blood glucose and high-sensitivity C-reactive protein, a marker of inflammation.
The authors concluded that long-term or high-dose honey consumption warrants caution in people with existing health concerns. Notably, the same review found royal jelly and propolis performed better across those measures than honey itself.
That is one review, and umbrella reviews inherit the limitations of the studies beneath them. It is enough, though, to retire the idea that honey is a health food you can add freely.

The gap between honey and table sugar in a cup of coffee is much smaller than the gap between sweetened coffee and black coffee. If a change is worth making, that is the one.
The Weight Loss Claim
This is usually what brings people to the combination, so it is worth answering directly.
Black coffee genuinely suits weight management. It has almost no calories, caffeine modestly increases energy expenditure and can blunt appetite for a while, and a cup of coffee often replaces something else.
Adding honey works against all of that. It puts calories and sugar back into a drink whose main advantage was having neither. There is no mechanism by which honey in coffee promotes fat loss, and no trial showing it does.
If weight is the goal, the useful version of this ritual is the coffee without the honey. If you simply prefer the taste with a little honey, that is a perfectly reasonable choice—just make it for the reason it is actually true.
Ayurveda and Heated Honey
Ayurveda takes a firm position here that is worth knowing about, because it cuts directly against putting honey in hot coffee.
Classical texts hold that honey should not be heated, and that heated honey becomes heavy, difficult to digest and productive of ama—the accumulated residue that Ayurveda regards as the origin of much illness. Honey is traditionally added to warm rather than hot liquids, and never cooked.
Modern food chemistry offers a partial parallel. Heating honey does increase hydroxymethylfurfural, a compound that forms when sugars are heated and which is used as an indicator of honey quality and storage. Whether the amounts formed in a cup of coffee matter to health has not been demonstrated, and the same compound occurs in many cooked and stored foods.
So the practical advice is easy either way: let the coffee cool for a minute before stirring honey in. It costs nothing, it respects the traditional view, and the honey dissolves just as well.
If You Want to Drink It Anyway
- Keep it to a teaspoon. The adverse associations in the umbrella review appeared at around 10 grams a day, so less is genuinely different from more.
- Let the coffee cool slightly before adding honey, for the reasons above.
- Count it. Honey in three cups a day is a meaningful amount of added sugar arriving without being noticed.
- Choose raw, unfiltered honey if you want the trace polyphenols, since heavy processing removes some of them.
- Do not give honey to infants under twelve months under any circumstances, because of the risk of infant botulism.
Who Should Be Careful
- Anyone with diabetes. Honey raises blood glucose. The glycated hemoglobin finding above does not change the fact that honey is sugar, and it should be counted as such.
- Anyone managing blood pressure, given the association with higher systolic pressure in the review
- Anyone with liver concerns, given the association with a raised liver enzyme
- Anyone sensitive to caffeine, or with a heart rhythm problem, anxiety, reflux, or difficulty sleeping
- Pregnancy, where moderate caffeine limits usually apply—worth confirming the current advice with your own doctor
- Infants under one year, for whom honey is unsafe
Where This Fits in a Wellness Routine
Coffee is one of the few everyday pleasures that the evidence treats kindly, and there is no need to complicate it. Drunk black, in moderation, it is associated with good outcomes across a great deal of research.
Honey is a lovely ingredient with a long traditional history and a genuine place in food. It is not a supplement, and the most recent pooled evidence suggests it should be used the way you would use any sweetener: sparingly, deliberately, and counted.
